NURSING CARE PLAN FOR CONSTIPATION

PROBLEM S> Hanak pay nga tinmaktaki manipod di kalman O> Distended abdomen noted > Firm abdomen noted > With hypoactive bowel sounds > Decreased bowel sounds EXPLANATION Diet, exercise, and daily routine are important factors in maintaining normal bowel patterns. Too little fluid, too little fiber, inactivity or immobility, and disruption in daily routines can result in constipation. OBJECTIVE STO: After 8 hours of nursing interventions, the patient will be able to demonstrate behaviors or lifestyle changes to prevent recurrence of problem such as: > Fluid intake of atleast 2-3 liters per day. > High in fiber diet > Activity and/or exercise as tolerated LTO: After three days of nursing interventions. The patient will be able to establish or regain normal pattern of bowel functioning. Nursing Diagnosis: Constipation related to decreased peristaltic activity secondary to immobility. INTERVENTIONS > Assess usual pattern of elimination; compare with present pattern. RATIONALE > It is important to ascertain what is normal for each individual and for comparison. > Change in mealtime, type of food, disruption of usual schedule, and anxiety can lead to constipation. > Prolonged bed rest, lack of exercise, and inactivity contribute to constipation. > Some individual who travel or require hospitalization may have difficulty having a bowel movement away from home. > To help strengthen abdominal muscles that facilitates defecation. > To promote passage of soft stool and helps to EVALUATION

>Evaluate usual dietary habits, eating habits, eating schedule, and liquid intake.

> Assess activity level.

> Assess privacy for elimination

> Assist in performing activity.

> Encourage to increase fluid intake as tolerated.

improve stool consistency. > Encourage to increase fiber in the diet. > Fiber is an important element in promoting healthy digestion. Fiber helps to add bulk to stools, making them easier to pass through the bowel. > Ambulation and/or abdominal exercises strengthen abdominal muscles that facilitate defecation.

> Encourage physical activity and regular exercise within the limits.
