Technology Certificate Project #7: Use Form to Create a Survey, Share Online Part I

For this assignment you will create a survey to send to your peers. In Project #9, you will use the information gathered to graph their responses. 1. Write five questions you want to ask your peers in a survey. You will use their answers to create a graph in project 9. Questions cannot be open-ended. This means the person could answer anything they want. You need to write answers for peers to choose from. For example: What is your favorite food? This could be a good question as long as you give them choices to pick from. Otherwise the questions is open-ended and you wont be able to graph the responses. If you ask them where they are from, you will have to give them choices. To cover people who are not from the places you mention, add the choice: other. If you ask about sports they participate in, include options of I just watch or I dont like sports so everyone can answer. Questions might include:
Where are you from What city do you live in Why are you in the ABE class i. Self-improvement ii. GED preparation iii. CAHSEE preparation iv. For credit toward my high school diploma v. Socialization vi. Other What month were you born in(must give 12 choices) What is your favorite type of food What are your future plans? a. Go to 2-year college b. Go to the university c. Go to a trade school or vocational class d. Get a job e. Pass a test so I can go onto A) a class or B) job requirement f. No immediate plan What is your favorite sport/ sports team, music, performer 2. To create your survey using Form: Open your g-mail account. Click on Drive at the top menu

bar. Click on Create on the left side. Click on Form. Give the survey a title [1] 3. Write any instructions concerning your survey. [2] Please answer these questions as soon as possible. I am doing this for a class assignment at school. Thank you. 4. Begin writing your survey questions. Try different formats for the answers: multiple choice, text, check boxes, choose from a list etc. [3] To get more questions, click on add item at top. [4] Write a minimum of 5 questions.

5. After you have entered all the questions you want to include in your survey, you will want to send it to the people you want to survey. There are several ways to do this, for this project, click on Email this form [5] 6. In the To box, you can type in the emails of those you want to send this survey form to or you can choose from your contacts list. [6] 7. Click on Send. [7] You will need a minimum of 10 people to share your survey with. (include the teacher) 6
