    The purpose and other conditions indicated above shall be strictly followed. If there are deviations, said changes shall be confirmed/certified by the official responsible for the travel. After the travel submit accomplishment report certified by the employee making the travel and approved by the Division Chief concerned, certificates of appearance, tickets and other evidences of travel. In case of plane tickets boarding pass shall be attached. For compliance.
