CSC REFERRAL PROCESS

Parent/Teacher identifies child who is believed to have a disability. Teacher has documentation of how suspected disability is affecting school performance. Make sure parents are aware of your concerns.

Teacher completes CSC Referral Form on the common drive and emails form to Valerie Sutherland. Form must include documentation of interventions used with the student and the students response to those interventions.
SPED team discusses pre-referral at team meeting. Meetings are held once/month or more as needed.

CSC team recommends further interventions for student and/or recommends student to SST.

Classroom teacher and/or referring individual is notified if the pre-referral will go to referral.

The CSC tech will schedule an assessment planning meeting within 10 school days.

Assessments are planned at the assessment plan meeting and parents must sign consent to assess. CSC has 45 working days to complete testing after parent signs permission to assess.

Once assessments are complete, we have ten working days to schedule an eligibility meeting.

Eligibility meeting is held.

If student is eligible, we must schedule a meeting within 10 school days to write student IEP.

If student is not eligible, records of testing will be kept in CSC office. Team will discuss regular ed. programming that is available to student.

General education and special education teachers should meet prior to IEP meeting to consult on appropriate goals and objectives.

IEP Meeting is held. IEP is implemented.
