SAFETY PLAN FOR LAYING OF CABLE TRAYS AND RELOCATION OF BOILER PANEL
PROJECT: LAYING OF CABLE TRAYS AND RELOCATION OF BOILER PANEL

LOCATION: F&HC BOILER ROOM

WORK SUPERVISION 1. The proposed installation will commenced 8th November, 2012 2. Supervision will be done by the project team: Project Technician. ADDITIONAL CONSIDERATION AND DISCIPLINARY MEASURE: 1. 2. 3. 4. 5. 6. Work will start at 8am and end 5pm; there will be no work beyond 6pm on site. Working hours for all Sam-electric staff is 8hrs. One hour break will be observed between 1pm and 2pm. Additional permits e.g. Hot work, scaffold permits will be raised as at when due. There will be provision of temporary pass during the weekend. There will be a dedicated safety officer from the contractor company on site during the project. 7. If any of the contractors workers is observed not complying with safety in any way, depending on the gravity of his noncompliance, the contractor safety officer will issue a verbal warning to him for the first time. The violator will get a written warning letter for the second time violation. If the same violation is repeated for the third time, this worker will be dismissed from working in the plant. 8. All permits approval must be done by HSE central. 9. If it is observed that the contractors site management is not showing the proper level of compliance with safety, a documented verbal warning will be given to the contractor site leader by the project manager, if the same violation is repeated for the second time, the Site safety group will have the right to stop the work and have a dialogue with purchasing where the final action will be concluded. Consequence resulting from the stopped work will be the contractors responsibility. 10. The scaffold to be used will be duly certified by the P&G safety officer.

11. First aid box would be provided and first aiders to be on alert, for immediate administration of first aid before transport to hospital. 12. Logistics for emergencies to retainer clinic shall be provided by contractor.

POSSIBLE CAUSE(S) Housekeeping not properly done. Worker not wearing body harness, not properly anchored to approved anchor points, worker using defective body harness, safety belt & not wearing hand glove.

SAFETY MEASURE/MITIGATION Orderly arrange all material and tools in the work area. Workers must wear Hand gloves, helmet, safety boot, safety belt. PPE inspection will be done every day.

Adhere strictly to the panel isolation procedure. Only competent personnel should perform task. Adequate supervision. Obtain Electrical work permit. Only competent scaffoldist should erect the scaffold. Obtain yellow tag permit to erect; and green tag after scaffold is inspected & approved. Workers must wear Hand gloves, helmet, safety boot, safety belt. PPE inspection will be done every day. N.B: For all task, no lone worker is ALLOWED. Deploy this risk analysis to workers before starting the job. Housekeeping MUST be on going. Workers MUST have toolboxes with them at work locations to avoid tools litter work area. Thorough inspection of all body harness and lanyards must be carried out before any work at height starts.

Panel and Cable Isolation Procedure Wear electrically gloves. Switch off the main circuit breaker Confirm that the outgoing of the circuit breaker is dead by using a line tester or voltmeter. Use electrically rated screwdrivers to loosen the terminals of the cables Remove the isolated cable from the cable rack gently. Remove cable glands or all cable ties with the use of an electrically rated side cutter. Pull the cable along the tray
