Asymmetric, bizarre, irregular hair loss pattern Circular, discrete patches of loss; short, broken hairs; exclamation point hairs at periphery of patches Patchy hair loss that appears moth-eaten Pruritus, erythema, and scaling

Possible Causes
Trichotillomania Alopecia areata

Secondary syphilis Chronic cutaneous lupus Tinea capitis (particularly if adenopathy present)

Pustules

Scarring dermatologic or infectious process (eg, dissecting cellulitis of the scalp, acne keloidalis nuchae) Alopecia universalis Primary hair shaft abnormality Adrenal disorder or tumor Polycystic ovary syndrome Anabolic steroid use (sometimes surreptitious)

Scalp and body hair loss Unruly or unusually wooly hair Virilization
