    List of question from the nurse to diagnosed Mrs.K problem : Hello, What's your name ?... How old are you?... How long your pregnancy?... How's your pregnancy?... Why do you think your appetite has decreased?... Before pregnancy, how much your weight?... How much your weigh during pregnancy?... Are you sick any other time?... When you were eating normally what sort of food did you cook?... Can I check for your vital sign?... How much your vomit a day?... Do you feel weakness?... Do you want to check your urine to take laboratory test?... With whom do you life?... Are you have a good social life with your neighbor?... Do you have much time with your husband?... Are your mother have a severe vomit and nausea during pregnancy too?...

    Discharge Planning: a. Treatment: Continued intravenous therapy with vitamins and thiamine, medication: ondasteron (Zofran) to decrease nausea and vomiting. Mrs. K still have nausea and vomiting. b. Plan of care: Continuous IV therapy (Teach Mrs. K how to prime the tubing, inject the vitamin/thiamine, and inspect the IV insertion for signs and symptoms of infection. c. Latest physician order: encouraged Mrs. K to to range of motion exercise while in bed, encourage Mrs. K to remain in the first level of home to avoid falls. d. Continuously monitor vital signs.
