GETTING DOCUMENTS FOR COLLEGE

STEP 1
A Release of Records Form signed by the parent or guardian must be on file in the Guidance Office. Due to federal and state legislation, the school must obtain written consent for release of student records for students under the age of 18. All required forms are available in the Guidance Office. Document Requests Forms: To request documents from the Guidance Office, complete the yellow Request for Transcript/Secondary School Report form and submit the form to the Guidance Secretary. Most requests will be processed for $1.00 and may require 2 weeks to complete. Students will be notified when documents are ready for pick up. Use a separate yellow Request for Transcripts/Secondary School Report form for each request. The transcript and secondary school report are provided with each request for documents. Students should list additional documents required on the request form. The Common Application Secondary School Report form, on file in the Guidance Office, replaces the individual colleges Secondary School Report form. Submit the green Student Profile form. The School Counselor references this information when completing the student evaluation section of the Secondary School Report. The Carver Centers School Profile, which describes the school and our students accomplishments, is also provided with each document request. In January/February, students may request transcripts with senior year grades by completing the pink Mid-Year Report Request form. In May, students are required to complete a post graduation survey and indicate where final transcripts are to be mailed. Final transcripts are mailed by the Guidance Office in late June. Teacher Recommendations: Request should be made directly to teachers. Request may be submitted on the Request for Teacher Recommendation form. A copy of the teachers recommendation may be filed in the Guidance Office. A Complete College Application May Consist of Four Pieces:

1. Student Portion includes the paper or online application form, application fee and any
essay(s).

2. Transcript and other documents portion are requested in Guidance and mailed by the
student. 3. Teacher Report is required by some colleges. 4. Test Score Reports many colleges require that official SAT or ACT test scores be sent directly from the testing agency. At registration, you may request four colleges to receive your score at no additional cost. Colleges accept AP scores from the School Guidance Office. Numbers 1 and 2 are required by all colleges. 3 and 4 may be required. PLEASE carefully follow the specific directions on each colleges application process.

Other Suggestions: Applying well before the deadline avoids the mistakes caused by stress. Photocopying or saving an electronic version of the application is a good idea. Develop a system where you list your colleges, deadlines and application requirements. Colleges expect to receive each piece of the application at different times and will build a file for each applicant and add to it. The use of the Common College Application may save time.
