PROJECT

This agreement relates to any and all materials originating from the interviews, namely
the video and or audio recordings of the interviews, photographs taken during the
interview, any items obtained from the interviewee, and any written material, including
but not limited to transcripts or other finding aids prepared from the tapes.

In consideration of the mutual covenants, conditions, and terms set forth below, the
parties hereby agree as follows:

1. Interviewee assigns to the Interviewer(s) all his or her copyright and title to the
video and audio recordings and transcribed interview.
2. By virtue of this assignment, Interviewers will have the right to use the interview
for research, educational, and other purposes, including publication in academic
journals, dissertation(s), books, and any future technologies that may become
available.
3. The product of this interview may be deposited in an archive at the interviewers
discretion.
4. The Interviewee retains the right to withdraw the interview from this project at
any time prior to the deposition of his/her transcribed interview in any archive.
5. The Interviewee will have the opportunity to review video recording and or
transcribed interview before it is deposited into any archive or used in any
publications and will be given the opportunity to request minor revisions,
additions, or deletions to the interview at that time.
6. The Interviewee will have the opportunity to receive a copy of the audio
recording or transcript for his/her own personal records.
7. I prefer the interviewer use my name or a pseudonym (PLEASE CIRCLE
ONE).
