Physical Examination Aldrin is lying on the bed, awake, alert, and cooperative.

Able to follow commands and is in no apparent distress although breaths rapidly. He is well built but he is a little bit malnourished. He appears closely bonded with his mother, looking at her occasionally for comfort. Vital Signs. Height is ....., weight ...., BMI ...... Chest circumference is 23.6 cms, abdominal circumference 21.2 cms. BP 90/70, right and left arms, HR 83 and regular, RR 48, temperature (axillary) 36.4C. Skin. Warm and dry. Normal color and turgor except for some ....... (things in his leg that you said. Old rashes???). HEENT. Head: Normocephalic; no lesions. Eyes: Symmetric with normal extraocular movements. Normal red orange reflex. Pupils were equally round and reactive to light and accommodation. Conjunctivae were pink. Sclerae anicteric. Ears: Normal pinna; no external abnormalities. Normal external canals and tympanic membranes. Nose: Normal nares; septum midline. Mouth: Moist and pink. No lesions. Tongue appeared midline in the mouth. Neck. Supple, midline trachea, no thyroid palpable. No neck vein distention or carotid bruits auscultated. No JVD. Lungs. Good expansion. Diminished breath sounds in the left lower lung fields. No accessory muscle use. No wheezes, no rhonchi, with good airflow. Good excursion. Cardiovascular. PMI in 4th or 5th interspace and midsternal line. Normal S1 and S2. No murmurs or abnormal heart sounds. Normal femoral pulses; dorsalis pedis pulses palpable bilaterally. Abdomen. No masses or tenderness with active bowel sounds. Liver span 2 cm below right costal margin (RCM) and not tender. Spleen and kidneys not palpable.
