Canyon View Elementary Yearbook Prideline Order Form

Pridelines are short congratulatory tributes written by you for your graduating 5th grader. The Pridelines are printed in the yearbook. There is no limit on Prideline purchases. Dont miss out! Three options to submit your Prideline - see below
All Pridelines must t in a 3 X 5 space (see below). The 3 X 5 Prideline will be published in the horizontal orientation and reduced in size. Return this form to the front ofce or teacher with you payment of cash or check payable to Canyon View FFO. There are three ways you can submit your Prideline. Check one of the option boxes below for your preferred option.

Option 1
Digital Prideline - (Best Quality) Email your Prideline tribute to Kerry Perlman kmcperl@aol.com. I will type up the tribute using a fancy font and a colorful background and digitally add it to the yearbook. If you make it more personal with a picture, please be sure you email the picture in the JPEG format. Your tribute with or without a picture must t in the 3 X 5 box below.

Option 2
Digital Prideline Dont have email but would like to have a digital Prideline? Simply write out the information in the 3 X 5 box below and I will type up the tribute using a fancy font and colorful background and digitally add it to the yearbook. If you would like to add a picture then submit one with your order and we can scan it to the Prideline.

Option 3
Scanned Prideline If you would like a handwritten message or just design your own tribute then write it out or type it in the box below or use your own 3 X 5 notecard. Make your tribute unique by using baby pictures, drawings, stickers etc.. Your message will be reduced in size and scanned into the yearbook. For handwritten messages please use pen or marker, no pencil.
