Team Registration Form

Thank you for your interest in participating in 80s Bowling! Below is an outline of
the details of the event (for more information see the event overview):
USC affiliated sororities are invited to enter a team(s)
Please fill out a registration form for each team participating
Each team will consist of a maximum of 5 people
The entry fee is $50 per team (10$ per person- includes Pikes Presents TShirt)
All checks should be made out to: Pi Kappa Alpha
This price includes shoes and 2 hours of unlimited bowling
There will be rides from the bowling alley to downtown afterwards
Please fill out the attached registration form and return the form and fees to a Pike
representative, Chris Wright, or to the Pike house.
