Medical Forms
These are to be extracted with the medical form template. Do not translate The printed form questions do not need translating.

Do translate (and add to the template) All handwritten answers and information on the form. This is to include (if they are given) : Name, Address and all personal details. Symptom details Treatment details Details of the medication prescribed

Invoices / receipts
These are to be extracted with the invoice / receipt template. Please use a separate template for each document. Do not translate

the format and layout of the invoice. logos and any other images information about terms & conditions, payment methods or other similar standard information from the invoice / receipt

Do translate (and add to the template)

Whether the document is an invoice for payment, or a receipt Who the invoice is from (Clinic name, hospital name, pharmacy) The breakdown of what services or medications are being charged for The total amount due Any amounts of discounts or pre-payments Any other handwritten or additional information Is the invoice / receipt confirmed as paid? By what method (Cash, card etc)

Prescriptions
These are to be extracted with the Prescription template. Please use a separate template for each document. Do not translate Logos, template, formatting and stamps.

Do translate (and add to the template) Name of the prescribing doctor / clinic All diagnosis info Names of medicines with quantities and treatment period information Details of any referrals or treatments prescribed (physiotherapy, scans, tests, re-education etc). If the prescription is stamped or marked to show that it has been dispensed by the pharmacy. Any other information specifically relating to the patient / consultation

The templates that we have provided should help you to understand what to extract and to work faster, so please do use them. Please ensure that you use the right template for the document type and use a separate template for each document. Please lay them out in the same order as the documents that are provided to you, so that the client can match them up.
