    PLAYER PERMISSION FORM

    FSMS Ping Pong Tournament


    WHEN: before school beginning Wed., February 12 to Friday, February 14. WHERE: FSMS (Aux Gyms) The PE Department will be hosting a ping pong tournament open to all 8th Grade FSMS Students. Games will be played in the auxiliary gymnasium. Games will begin at 730AM and finish at 815AM. Students are required to find their own transportation. All students who returned a signed permission form will be allowed to play. Signed permission forms or Parent/Guardian email permitting attendance are due to Mr. Treon by Tuesday, February 11th. NO EXCEPTIONS WILL BE GRANTED!! As always, good sportsmanship is required for this activity. Players failing to demonstrate proper behavior may be removed from the tournament. ________________________________________________________________________ (please detach and return to Mr. Treon) I give my SON/DAUGHTER (please circle) permission to participate in the before school ping pong tournament.
