This sheet should be filled out and signed as each area of community connection is completed. The adult in charge of the ministry, activity, or service project is the appropriate person to sign off on this form. When making your Mass connections, have a ministry mentor or catechist sign this sheet for you. The completed sheet is due to the Youth Office by May 31, 2014 in order to receive credit for the years course and to be able to register for next years class.

There are many volunteers who help the community worship during any liturgy. You can be an usher, lector, greeter, choir/band/orchestra member, or simply sit in the youth section at the 5:30pm Sunday Mass.

Social - For this area, attend 3 social events

Join your Epiphany friends for a little fun and fellowship in a number of different ways. You can come to any youth ministry sponsored event or activity such as JAM, retreats, half day lunch, concert, fun night, etc. You can also join the whole community when it gathers for various events like our concert with the Mormons in December or the Epiphanic in the Spring.

Service - For this area, participate in 2 service activities

There are many service opportunities throughout the year at Epiphany. There are also many projects that other small faith formation groups are involved with that you can join. You can find a project by logging on to www.epiphanyyouthministry.com. Once you are there, click on the Teens tab and then service opportunities. You will see a list of projects and the person in charge. Let that person know you are coming and have them sign your sheet.
