Isaac Observed February 4, 2014- February 11, 2014 Physical Assessment Sensory Integration On February 4th, Isaac was

determined at a level 4 for sensory integration. This is an infant/toddler standard. Isaac typically focuses on one or two activities throughout the day. These are areas where he is able to be in control. On February 4th, Isaac spent over an hour at the discovery table. Then, he chose to participate in art, where he was able to paint whatever he liked. In the picture below, Isaac decided that he wanted to play with the kitchen toys alone. He asked, Can I do this alone?

Stability Isaac is at level 4. This is an infant/toddler standard. One of Isaacs favorite activities is playing at the discovery table. On February 4th, Isaac used one hand to hold the different animals and used the other hand to move around the grass and moss. Isaac is able to stand independently at the table. Also, Isaac is able to balance his body in different ways without wobbling and falling down.

Gross Motor Skills Isaac is at a level 5 for his gross motor skills. On February 4th, Isaac used different methods to play limbo. He bent under the stick, slid, rolled, and walked on his hands and knees. Also, Isaac is able to run, skip, and hop around the room and outside with

ease and without falling. Below, Isaac is using both of his feet to hop across the room and alternating feet to skip.

Object Control Isaac is at a level 4-5 for object control. Isaac is able to throw the ball in a straight direction, but was unable to catch the ball. Isaac first missed the ball, on February 4th, while trying to kick it. After an additional try, he was able to kick the ball to the other side of the room.

Precision Hand Skills Isaac is at a level 2-3 for precision hand skills. On February 4th, when Isaac was asked to write his name, he held the pencil incorrectly and wrote his letters all over the page in random order. His name was difficult to read. Isaac enjoys participating in art activities. He is able to glue and paint with precision. When looking at Isaac cut his paper, he first held the scissors with his thumb down. However, after showing Isaac the appropriate way to cut, he finished cutting correctly.

Isaac Observed February 4, 2014- February 11, 2014 Personal Care Oral Motor Isaac is at a level 4 for this infant/toddler standard. He is able to chew his food with his mouth close and nothing falling out of his mouth. Also, Isaac uses his tongue to move around his food.

Self-Feeding Isaac is at a level 5 for self-feeing. He is able to serve himself using a pitcher and tongs. Also, on February 4th, Isaac was able to spread the jelly on his toast with a knife with precision. Then, he was able to pour the milk into the cup without spilling or help.

Dressing/Undressing On February 6th, Isaac was rated a 2-3 for dressing and undressing. Isaac needed assistance with zipping his coat, putting on his snow pants, and gloves. When asked to take his shoes off and on, instead of unfastening, Isaac kicked off his shoes and slid his foot back in.

Care of hands, face, nose On February 4th, Isaac was rated as a 3 for the care of his hands, face, and nose. Isaac did not have to be reminded to wash his hands before and after snack. He was able to wash and dry his hand completely and by himself. Also, when Isaacs nose was running, he walked over and grabbed a tissue. He then blew his nose and threw away his Kleenex by himself.

Toileting On February 6th, Isaac was rated a 3 for toileting. He does not need reminders to use the restroom. He is able go to the bathroom, flush, and manage his clothing. Then, Isaac washes his hands, uses soap, and dries his hands by himself. He did not need any additional help or reminders about flushing the toilet or washing his hands.

Isaac Observed March 18, 2014- March 27, 2014 English and Language Arts Demonstrates Awareness of Sounds On March 18th, Isaac was rated a 4 for his ability to demonstrate awareness of sounds. When asked what rhymed, Isaac said, rhyme and phyme then, he was asked, what rhymes with cat. Isaac responded, spat. Isaac is able to understand what it means to rhyme, but is unable to find actual words that rhyme together. Next, Isaac is able to match other words that begin with I. He said, ice and icicle. Isaac was also asked to name a word that begins with the letter c. Isaac said, cat starts with a c. Demonstrates Awareness of Symbols Isaac is a 3 for his awareness of symbols. On March 18th, I showed Isaac different logos on the iPad. He recognized McDonalds, Pizza Hut, and Angry Birds. Also, Isaac was shown different actions, including running, swimming, and skating. He recognized all of these actions correctly. Then, Isaac was asked of the letters in his name and accurately listed all five letters that are in his name and was able to recognize them on paper. Uses Print for Pleasure and Information Isaac is at a level 4 for uses print for pleasure and information. On March 25th, when asked where the words were in the book, Horton Hears a Who, Isaac was able to point to the title and author of the book. Next, he was asked to point his finger to the words as I read. He was successfully able to follow words from left to right and top to bottom. However, Isaac is not yet able to tell a story while looking at a book. He said, Um, I dont know what it means.

Comprehends Details, Events, and Main Ideas On March 25th, Isaac was rated at level 3. After reading Horton Hears a Who, Isaac was able to describe the front of the book cover. He said, It is orange with an elephant. Also, while reading, Isaac was able to tell about the pictures in the book and make a guess as to what was happening in the story. Isaac said, Horton is holding the flower where the people are. He was able to identify characters in the

book along with main themes. However, Isaac was not able to tell simple stories from the books or retell the beginning, middle, and end of the story.

Writing for a Specific Purpose and Audience On March 27th, Isaac was rated at 3. He was able to draw pictures to express his ideas and tell a story. Isaac was asked, What do you see on your way to school? He said, Every single day, I come over to school. I pick a spikey ball from the spikey ball tree. He used his picture to help illustrate his story and told his story for a teacher to write.

Uses Writing Implements Isaac is at level 4. He not able to write his name on one line from left to right. He is able to produce all of the letters in his name, but does not write them in the correct order, as seen in the picture below. Also, Isaac was able to write numbers from 1-5. However, the number 2 and 3 were the incorrect direction. On March 18th, Isaac wrote an O and n, which is how he writes his brothers name, Orin. Below shows pictures of Isaac writing his name along with his brothers name. As you can see, the letters are not in the correct order.

Demonstrates Receptive Language Isaac is at the level 5 for this section. On March 18th, Isaac was able to follow the unfamiliar directions given to him. He was told, Isaac in five minutes, you need to come and do small group. He responded, ok and a few minutes later Isaac came to small group. Also, on March 18th, Isaac was told, Please go to your cubby and then go put your coat on. It is time to go outside. He followed these instructions correctly and appropriately. From the picture below, you can see that Isaac needed additional help putting his coat on.

Demonstrates Expressive Language Isaac is at a level 4-5. On March 18th, Isaac was asked, What did you do over spring break? He said, Uh, went to, uh, I forgot. Then, he was asked to tell about his brother. He said, He is in kindergarten. He likes to play, but doesnt like to do his homework. Isaac is able to produce 6-word sentences, but does not use multiple descriptor words and struggles with grammatical rules. Isaac is able to tell his name, age, and gender upon request. Finally, Isaac was asked to count along with the teacher. She started with 1, 2 Isaac was able to complete the number chain with saying, 3, 4, 5.

Isaac Observed March 18, 2014- March 25, 2014 Social Emotional Sense of Self and Others On March 18th, Isaac was rated at a level 4 for his sense of self and others. When asked the name of his sibling, he correctly responded with, Orin. Also, Isaac engages with others on a regular basis, especially at the discovery table or dramatic play. Below is a photo of Isaac working with Etienne at the discovery table. Isaac is respectful of himself and others by asking if he can help, following directions, and listening. On March 18th, Isaac encourages others by telling Aine, the blocks look cool. Also, on March 20th, Isaac asked if he could sit by himself and read a book. He enjoys working independently on certain tasks.

Manages Emotions Isaac is at a level 4. On March 18th, when asked how he deals with his emotions, Isaac said, I cry when I am sad. I like to play with daddy. From this, one can see that he manages his emotions and relies on the comfort of his dad. Isaac does not show aggression towards his friends, but instead makes compromises. On March 18th, Isaac was asked to come and do small group. He did not want to, so said, I do not want to. Can I in five minutes? Interpersonal Skills On March 18th, Isaac was rated as a level 3 for his interpersonal skills. He was playing at the discovery table with three others friends. Andrew came up, so Isaac responded to the adult guidance and took a turn with Andrew. On March 20th when Isaac came to school with his dad, he greeted his friends and teachers with a smile and short, hi. On March 20th, Isaac played in the rainforest. He pretended to be on an adventure with his backpack on. Responsibility Isaac is at a level 4. Isaac follows the rules and routine of the classroom. When asked to clean up, on March 18th, Isaac helped clean up the art table and then helped Aine and Nicky finish cleaning up the blocks. Isaac regularly works independently at the discovery table. It appears that he enjoys when he is alone at the discovery table. Isaac also respects his teachers and friends by asking to borrow

a toy or move on in an activity. On March 18th, Isaac asked if I would accompany him to the science room. Finally, Isaac was unable to repeat his address or telephone number.

Problem Solving Isaac is at a level 3 for his problem solving skills. On March 25th, Isaac asked Miss Probst to help put his smock on. He said, Miss Probst, can you help with my smock? He was able to explain why he needed to wear his smock and its importance. Then, he was asked what would happen if he did not wear his smock. He said, I would get messy and in trouble. He was able to make predictions as to what would happen if he did not follow the rules.

Approaches to Learning Isaac is at a level 3. On March 20th, Isaac played at the discovery able for a total of one hour. This is his preferred activity when he is at school and he is able to maintain attention while playing for that period of time. Also, Isaac typically has a positive attitude when asked to try a new activity. On March 20th, Isaac participated in the birthday party for everyone. This was an activity new to Isaac and he asked, Why are we doing this? While doing art on March 18th, Isaac worked neatly and carefully to craft his butterfly. He stayed at the art table until his butterfly was completed and he was satisfied.

Isaac Observed March 18, 2014- March 27, 2014 Math Counting and Quantity On March 18th, Isaac wrote the numbers one through five on the dry erase board. However, the number three was written as the letter e. Then, he was asked which is the most. Isaac said, five. Next, he was asked which of the numbers is the least. Isaac responded with, one. On March 20th, Isaac was able to count from 1-25. However, he skipped the numbers 16 and 20. When he was asked about 16 and 20, he said, Oh yeah, and 16 and 20. Next, when Isaac was working with the foam blocks, he was able to count 10 of the blocks easily.

Computation Isaac is at a level 4. On March 20th, Isaac was able to add two rectangles and two triangles. He correctly answered with the number four. Also, Isaac is able to successfully use the terms, whole and half. Next, on March 27th, Isaac was able to sort the animals by their size. He made a pile for the smaller animals and a pile for the larger animals. Also, he was asked, How many pigs will we have if we take one away from the pile of 4? He answered, 3.

Time

On March 27th, Isaac was rated as a 4. When asked, Is it morning or afternoon right now? Isaac answered, I think it is morning. He correctly used the concepts of morning and afternoon. Also, Isaac was able to tell what he did

yesterday and what he is doing today. He correctly used these terms. Finally, I asked Isaac what happens at school. He said, We have morning meeting, snack, and circle time. Sometimes we go outside. Isaac was able to sequence these events and tell the routine of the day. Location Isaac is at a level 5. On March 20th, Isaac was asked, Where are the bicycles? Isaac said, Theyre outside. Next, he was asked, Are the blocks kept inside or outside? Isaac responded with, inside. He is correctly able to use the prepositions inside and outside. Next, Isaac was able to use his communication words to tell me where the discovery table is and where the Kleenexes are kept. Length, Capacity, Weight, and Temperature On March 27th, Isaac was rated as a 3-4. He was able to measure which line was longer on the page of paper. However, when asked how long it was, Isaac answered 9 inches, instead of the correct answer of 6 inches. Isaac was able to successfully order three blocks by size, as seen below. Also, Isaac is able to recognize heavy objects verses light object. Next, at snack on March 27th, Isaac was able to see which objects were warmer and cooler, by answering, Orange juice is cooler than bread.

Sorting and Classifying Objects On March 20th, Isaac rated as a level 5. When given the foam blocks and told to sort them, Isaac began sorting the blocks into different colors. Then, Isaac sorted the blocks by their shape. Next, Isaac was asked to make a pattern with the foam blocks. He made a pattern of square, rectangle, square, rectangle. Also, Isaac observed that some of the blocks could fit inside other blocks. So, he experimented with how blocks fit together and which blocks cannot fit together.
