Contact Information: Insurance, physician and emergency contact
Registration Fee: ($145/1st child, $120 each additional child),
Two Deposit Checks: uniform ($50 per child) & snack stand and field duty ($200 per child)
Copy of Live Birth Certificate – the Abstract is not acceptable
Copy of utility bill or other proof of residence within boundary
