Actor permissions

This is a contract stating that you will abide by

the production schedule of the video shoot and
conduct yourself in a professional manor to
ensure the safety of yourself and those around
you. By signing this contract you are agreeing to
the health and safety risk of participation in the
video shoot. You are also giving us permission to
use your image in this Music Video and any
other advertisements such as posters or flyers.
