    Requirements:
    1. Latest pass port size colour photographs 12 Nos.
    2. Demand draft for Rs.1,37,500/- (Rupees one lakh thirty seven thousand five hundred only) drawn in
    favour GITAM Institute of Pharmacy payable at Visakhapatnam towards tuition fee for the first year B.
    Pharmacy. The amount can also be paid in the form of cash at the bank counter in the GITAM University
    campus, by a challan.
    3. SSC/CBSE Marks statement / ICSE statement of marks & Pass certificate (10

    4. Intermediate pass certificate cum Memorandum of marks (10+2 science).
    5. Transfer certificate from the Institute last studied.
    6. Study & Conduct certificate from the Institute last studied.
    7. Integrated caste certificate in case of SC, ST from a competent authority.
    8. Medical certificate from a registered Medical Practitioner not below the rank of a civil surgeon in case of a
    PH candidate.
    Note: Documents in respect of Sl. 3 to 8 are to be submitted in original along with one set of photocopies.
