1. Accomplish this application form and pay the non-refundable (P375.00) testing fee at the Cashier’s Office.
2. Submit the accomplished form with 2 ID pictures (1 ½ X 1 ½) to the Office of Admissions and secure your test permit.
3. Take the TUP Admission Test on the specified date, time, and room. Bring with you your test permit and 2 pencils with eraser.
4. Release of test result will be posted on ________________ at the CAS Building lobby or visit our website www.tup.edu.ph.
5. Report on the specified date for your interview and submit the following documents for enrollment.
a. Test permit
b. Original copy of Form 138/137 or transcript of Records
c. Certificate of good moral character
d. Xerox copy of Birth certificate
*Failure to report on the specified date will mean forfeiture of Admission slot.
6. Secure your Notice of Admissions and proceed to the University Clinic for medical examination
7. Report to your respective course adviser/dean for enrolment.
Note:
• Applicants for Engineering, Architecture and Science courses should have 80% grade or better in Math, Science and English.
• For transferee applicants, no failing grades and may enroll only in 3 year diploma courses.
• Enrollment will be on a FIRST COME FIRST SERVED basis.
