Science and Technology, Kochi-682022 as to reach on or before 13-5-2013.
1. Print the application form from the Completed Form, affix the identical passport size photographs in the space provided, put
your signatures at the required places and send it to the Registrar of Cochin University of Science and Technology, Kochi-
682022.
2. Copy of Bank Challan (UGC copy).
3. Attendance Slip duly signed with photograph (One copy).
4. Attested copies of SC/ST/Persons with Disability (PWD)/ OBC (Non creamy layer) certificates, in case applicable.
5. Certificate of educational qualification/research experience, in case applicable, entitling the candidate for age relaxation, duly
attested by a Gazetted Officer.
