Referral Process

1. Identify a student in need?


2. Look in the student le for
previous notes and referrals.
See Well Being Leader. Look on Server in Student
Proles - Current year les. Check Wellbeing Folder
for the Speech list, Funding Review List, LNSLN list,
Problem Solving List and if not on any list add the
Problem Solving List.

3. Record information on
Problem Solving Form/s on
Server. Learning Styles,
strengths, challenges.

4. Collect and date all testing
data, work samples,
observations, evidence of
student work, video, sound
recordings, collegial
observations. Talk to parents
(record this conversation on a
ROC - Record of Oral
Conversation - and le) for
other information including
eyes, ears, family.
* Use the Internal Referral
Request Form to begin.

trial strategies for
4-6 weeks,
reassessing for
