DISCHARGE PLANNING

Medication

Take pain reliever (Paracetamol) as needed.
Take Cefuroxime for prevention and treatment of infection.

Environment
Diminish infection rate by : proper waste disposal, control stray animals,
provision of people from bathing in infested streams, provision of
adequate water supply for bathing, laundering and safe water for drinking.
Keep the patient's room at a comfortable temperature. Check the patient's
skin temperature by feeling the extremities for warmth or coolness. Adjust
the room temperature if the patient's skin is too warm or too cool.
Provide a safe environment because injury may occur secondary to
decrease muscle function and seizure attack.
Encourage to have a clean and stress-free environment.

Treatment
Take the proper medications at the indicated times.
Explain action of medication, side effects and drug interaction.


Health Teachings
Provide education on the disease process, mode of transportation and
prevention.
Advised SO to stay with the patient and protect him from further injury
during seizure. Informed SO that if patient is standing or sitting during
seizure attack, patient should be eased up onto the floor, protect the head
with small pillow or place head on lap. Instructed not to apply restraints.
Loosen constricting clothing especially around the neck. Maintain airway
by turning patient on the side to drain saliva. Do not force to open jaw
during seizure
Provide measures to increase mobility. Refer patient to physical therapy (
passive or active ROM ). To stimulate or improve muscular functioning &
stimulate blood circulation. Use of assistive devices like crutches, cane
and walker.


Out Patient Department
Instruct the patient to visit the physician as when he was notified for a
follow up check-up.
Advise the patient to tell the physician for any concerns and if there are
any problems encountered.
Educate the patient the importance of a follow up checkup.

Diet
Provide well-balanced diet, high in fiber to prevent constipation.
Limit fluid intake to 1,200 to 1,500 mls/day to reduce CSF production.


Spiritual
Reassure patient that this is not a punishment from God.
Advise the patient to continue her healthy relationship with God.
Remind the importance of prayers.
