The undersigned student, who was considered as ON PROBATION AND/OR WITH
UNRELEASED GRADES during the previous semesters, understands, accepts and hereby
bind him/herself that pending the submission of the other final grades, his/her enrollment in
the SECOND SEMESTER OF SCHOOL YEAR 2014-2015 is CONDITIONAL UPON ACTUAL
COMPLIANCE with the Rules on Scholastic Responsibility, and that in the event when the
final grades have been submitted and it turns out that the undersigned student was not
allowed admission/enrollment for the SECOND SEMESTER OF SCHOOL YEAR 2014-2015, then
the undersigned students enrollment shall be terminated and his/her continued attendance
in the courses shall strictly be on an audit no-credit basis, even in the event an appeal is
interposed on any final grade obtained.
