GENERAL RELEASE OF LIABILITY

By the very nature of the activity; birthday parties, open gyms and other events at our facility (MEGA) carry a risk of physical injury. No matter how
careful the student & coach are, no matter how many spotters are used, no matter what height is used or what landing surface exists, the risk cannot be
eliminated. Risk can be reduced but never eliminated. The risk of physical injury includes minor injuries such as bruises & more serious injuries such as
broken bones, dislocations, and muscle pulls. The risk also includes catastrophic injuries such as permanent paralysis or even death from landings or
falls on the back, neck, & head.
The undersigned parent or guardian hereby releases, acquits, & forever discharges Missouri Elite Gymnastics Academy (MEGA) &/or any team, group,
or organization that is formed or associated with them of & from all liability to & demand of the undersigned & said student in respect to any accident if
injury which may result in connection with attendance or participation in classes while enrolled at MEGA. I hereby give my permission for my child to
be given first aid & CPR by the trained staff & to be transported to the nearest doctor or hospital in case of an injury or illness when unable to locate
parent or guardian in a timely fashion.
