Classes of
molecules
mediating and
regulating angiogenesis
There are today hundreds of molecules known to mediate or regulate angiogenesis. They can be
grouped in the seven classes a depicted here. Many molecules have been considered as potential
therapeutic targets or tools to inhibit pathological angiogenesis, in particular tumor angiogenesis.
Integrins in angiogenesis
Preclinical and clinical evidence indicate that vascular integrins may be valid therapeutic targets. In
preclinical studies, pharmacological inhibition of integrin function efficiently suppressed angiogenesis
and inhibited tumor progression. alphaVbeta3 and alphaVbeta5 were the first vascular integrins
targeted to suppress tumour angiogenesis. Subsequent experiments revealed that at least four
additional integrins (i.e. alpha1beta1, alpha2beta1, alpha5beta1 and alpha6beta4) might be potential
therapeutic targets. In clinical studies low molecular weight integrin inhibitors and anti-integrin
function-blocking antibodies demonstrated low toxicity and good tolerability and are now being tested
in combination with radiotherapy and chemotherapy for anticancer activity in patients.
Integrin Signaling
Signaling pathways initiated by integrins at focal contacts. The four major signaling pathways
activated by integrin engagement in adhesion complexes are shown. The key element in all these
pathways is FAK, which becomes activated through autophosphorylation at Y397 and thereby allow
binding of Src and Fyn for further phosphorylation and full activation. Phosphorylation of FAK at
specific sites dictates its subsequent interactions with other proteins (i.e. Grb2, p130Cas, PI3K, Graf)
which in turn elicit a cascade of events that lead to cell proliferation, migration or survival. FAK can
also be activated by cell surface receptors for growth factors, hormones and chemokines. Plain lines,
direct activation or inhibition; dashed line, indirect functional interaction; red lines, FAK-mediated
events mediated by specific phosphorylation events.
Diseased or injured tissues produce and release angiogenic growth factors (proteins) that
diffuse into the nearby tissues.
The angiogenic growth factors bind to specific receptors located on the endothelial cells
(EC) of nearby preexisting blood vessels.
Once growth factors bind to their receptors, the endothelial cells become activated. Signals
are sent from the cell's surface to the nucleus.
The endothelial cell's machinery begins to produce new molecules including enzymes.
These enzymes dissolve tiny holes in the sheath-like covering (basement membrane)
surrounding all existing blood vessels.
The endothelial cells begin to divide (proliferate) and migrate out through the dissolved
holes of the existing vessel towards the diseased tissue (tumor).
Specialized molecules called adhesion molecules called integrins (avb3, avb5) serve as
grappling hooks to help pull the sprouting new blood vessel sprout forward.
Additional enzymes (matrix metalloproteinases, or MMP) are produced to dissolve the
tissue in front of the sprouting vessel tip in order to accommodate it. As the vessel extends, the
tissue is remolded around the vessel.
Sprouting endothelial cells roll up to form a blood vessel tube.
Individual blood vessel tubes connect to form blood vessel loops that can circulate blood.
Finally, newly formed blood vessel tubes are stabilized by specialized muscle cells (smooth
muscle cells, pericytes) that provide structural support. Blood flow then begins.

Hipoksia merupakan rangsangan
terkuat ekspresi VEGF baik pada in vitro maupun in vivo.VEGF gene
promoters,mempunyai elemen cis-acting enhanceryang akan berikatan dengan
hypoxiainducible factor (HIF),suatu faktor transkripsi. Dengan ikatan pada reseptor
VEGF dan terjadi transauto-posporilirisasi dan aktifasi kaskade sinyal transduksi meliputi
jalur Src,PLC-g,MAPK, STAT3 dan STAT5. (11)
Ekspresi gen VEGF diregulasi
oleh suatu stimulasi pejamu, termasuk estrogen, nitric oxide(NO) dan berbagai variasi
growth factor, diantaranya fibroblast growth factor-4, PDGF, tumor necrosis factor
alpha(TNF-), epidermal growth factor(EGF), transforming growth factor beta (TGF-),
keratinocyte growth factor, interleukin (IL)-6, IL-1 dan insulin-like growth factor-1
(IGF-1).
Konsisten dengan perannya pada angiogenesis tumor, ekspresi VEGF
diregulasi oleh kejadian-kejadian genetik yang biasa terjadi pada transformasi maligna,
seperti hilangnya gen-gen supresor tumor (misalnya p53) dan aktivasi onkogen seperti
ras, v-src dan HER-2.Selebihnya ekspresi VEGF secara khusus lebih sensitif terhadap
tekanan oksigen dan sangat terpengaruh oleh hipoksia yang menjadi karakter hampir
semua tumor.
Dengan VEGF, hipoksia juga menimbulkan regulasi VEGFR-1 dan
VEGFR-2 pada sel-sel endotelial. Peningkatan ekspresi gen-gen VEGFR juga diinduksi
oleh pengikatan kepada VEGF itu sendiri, sehingga menimbulkan amplifikasi sinyal
VEGF. TGF-, yang juga menimbulkan peningkatan VEGF, telah ditemukan menurunkan
regulasi kadar VEGFR mRNA pada sel-sel endotelial, sedangkan TNF- dilaporkan
mempunyai efek positif dan negatif terhadap ekspresi VEGFR-2.
Reseptor VEGF
Sinyal kunci yang mengatur pertumbuhan dan diferensiasi sel, juga regenerasi dan
remodeling jaringan dewasa, dilakukan melalui reseptor transmembran, dimana sebagian
besar adalah reseptor tyrosin kinase.Reseptor VEGF, VEGFR adalah suatu resptor
kelompok tirosin kinase.
Ligands VEGF menghantarkan efek angiogenik melalui ikatan yang spesifik dengan
VEGF reseptor (VEGFR). Ikatan tersebut mengakibatkan perubahan konformasional
pada rseptor berupa dimerisasi dan berlanjut dengan sinyal transduksi melalui domain
tirosin kinase. Ada 3 reseptor primer dan 2 buah ko-reseptor yang akan mengikat VEGF
dan keluarganya, yaitu VEGFR-1, VEGFR-2, VEGFR-3 dan ko-reseptor Neuropilin-1
dan Neuropilin-2. Neuropilin tidak mempunyai domain/area intraseluler dan diduga
meningkatkan afinitas VEGF pada reseptor primer. Bebarapa variasi afinitas ligaands
pada reseptor dapat dilihat padagambar dibawah ini.
Reseptor transmembran vascular epithelial growth factor (VEGF) dan ligands
meliputi : VEGFR-1 (Flt-1), VEGFR-2 (KDR/Flk-1), and VEGFR-3 (Flt-4), neuropilin-1,
and neuropilin-2.
Ekspresi reseptor VEGF pada sel endotel berbeda diantara 3 jenis reseptor VEGFR-1,
VEGFR-2 dan VEGF-3. VEGF-2 diekspresikan hampir pada semua sel endotel,
sedangkan VEGFR-1 dan VEGFR-3 hanya diekspresikan pada pembuluh darah tertentu.
Sinyal trasduksi dari ikatan VEGF pada reseptor VEGFR-2
containing receptor) flk-1(fetal liver kinase-1),disandi oleh gen yang berlokasi pada
region 4q11-q13 , sebuah protein berukuran 230 kDa. Selama masa embrio, ekspresi
mRNA
VEGFR-2
terdistribusi
pada
kapiler,
pembuluh
darah
dan
pada sel endotel pembuluh darah ataupun pembuluh darah pada tumor.Ikatan pada
VEGF akan menyebabkan peningkatan migrasi sel endotel.NRP-1 dan NRP-2
keduanya adalah suatu protein reseptor transmembran yang berhubungan dengan
regulasi proses angiogenesis. Reseptor neuropilin akan mengikat beberapa isoform
dari VEGF meliputi : VEGF, VEGF-E,PIGF-2 dan VEGF-B. Penelitian terakhir
mendapatkan bahwa reseptor NRP-2 juga berikatan dengan VEGF-C dan
diekspresikan bersamaan dengan VEGFR-3 dan berperan dalam perkembangan
saluran limfe.
