Witness Interviews Jacob Brockman Incident

Heard someone holler.
Recognized that is was coming from the Roundo
Saw Jacob in the machine and knew he was in trouble
Kyle, Don and Tracy moved quickly to the Roundo
Tracy was already there and had stopped the machine by hitting the e-stop
Others arrived including Dwight
Someone asked for a pry bar
Kyle left to see if he could located a pry bar
Bubba lifted the upper roll allowing Jacob to be removed
Dwight began to assess Jacobs condition
Bubba went to activate EMS
I began to disperse employees that were gathering who didnt have to be
there.

Cleaning the rollers on the Roundo. Two grades of sand paper held from the
outward rolling side. The rolls are allowed to run.
Knows there is a procedure that requires the rollers to be stopped
intermittently while a section of the rollers are cleaned and the process
starts again
Upon completion Tracy left the rollers running.
Jacob followed up with alcohol that is applied to a shop towel used to wipe
down the rollers
Tracy went to the back of the TC500 to look over some paper work
Heard Jacob yell out
Moved quickly to the Roundo and found Jacob locked into the rollers. The
rollers were still moving
Tracy hit the E-stop which shut the machine down
Jacob could not self-extricate
Bubba and Don arrived
Don has a very good understanding of the roundo
Don started the machine back up and was able to raise the upper roller a
bit higher
Bubba began to lift on the rollers concurrently
Jacob was freed

Was at the new robotic arm when he heard someone cry out
He saw what was happening at the roundo
Don, Tracy, Dwight and Kyle were present
Jacob was still in the machine when he arrived
Don started the machine up
I began to lift on the top roller

We were able to remove Jacob from the machine

Marty was doing an admirable job in making sure the was room for people
to work
I left to make sure 911 was called
I ran into Mark Williams who then called 911

I arrived and I saw what was happening

I started the machine up and was able to lift the roller up a bit more
Bubba began to lift on the upper roller
This allowed Jacob to free himself
He was given a chair
Dwight began to assess his condition

I heard the hollering and I saw what was happening

I went to get Don because I know he knows that machine
I then went to get Joel
I heard there was a need for a pry bar so I tried to track down one
By the time I got to the machine Jacob was out sitting on a chair

Heard some hollering

I saw what was happening
I realize ERT members were on the scene
I went back to my office and called 911 and notified Dwight
Then went to the front of the building to await emergency medical
