Policy purchased by student

Student named as insured


Specifies coverage for health care professional liability, medical
malpractice or similar language
Policy limits (at least $1 million/$3 million)
US carrier or coverage extends to students actions in the US
Policy dates (covers students time at UW)
Policy basis (should be for claims arising from acts occurring during
policy period, not just for claims made during policy period) 1
Student agreement to notify UW regarding change in coverage (e.g.,
