Steps for Creating a Form for Camp

(Please Note: you will not have to create a week for September 1st-5th,
November 3rd-7th, November 27th-28th, and December 24th-26th)
1. Create a new form and change the name to your first name and Date(Aug
25th- Dec. 31st) (In Form Settings)
2. Add name and rename the field to Campers Name
3. Add Single Line Text. Change the name to Childs Age.
4. Add Single Line Text. Change the name to Email.
5. Add Multiple Choice. Change one to Soccer/Futsal and the other MultiSport
6. Add Multiple Choice. Change one to Returning Camper and the other New
Camper Registration.
7. Add Paragraph Text. Change the title to Please list the time your child
needs dropped or picked up if not in the normal camp hours.
8. Add Paragraph Text. Change the title to Please list any allergies or
medical conditions that we may need to know. Also, please leave any
questions for us as well!
9. Add a Page Break

Add Name and rename it Parent or Guardians Name

Add Single Line Text. Change it to Parents Email
12.
Add Date and Change name to Childs Birth Date.

Add Single Line Text. Change it to School Attended.
14.
Add Single Line Text. Change it to Current Grade.
15.
Add Address. Change it to Parent / Guardian Address:

Add Single Line Text. Change it to Emergency Contact.
17.
Add Single Line Text. Change it to Relationship.
18.
Add Phone. Change it to Phone Number.
19.
Add Phone. Change it to Alternate Number.
20.
Add Single Line Text. Change it to Insurance Provider and Policy
Number.

Add Single Line text. Change it to Where did you hear about
HQCamp?

Add Multiple Choice. Remove all the choice except for one. In the title
change it to Waiver. You will now type the waiver.

Add a Page Break.

Add Section Break. Change it to Selecting Days for Camp. Enters these
instructions into the You may select for the entire track, for a week(s) in the
track or for individual days. If you select individual days, we will provide the
week discounted price if you select five or more days in a given track.

Add Multiple Choice. Change it to The track I am signing up for is
(based on WCPSS Year Round Calendar):. Label the choices as the
follow: Track Two, Track One, Track 3, Track 4.

Add a Page Break.

Add a Section Break. Change it to Track Two.

Add Checkboxes. Change it to Track Two Individual Weeks ($195
per week). In each checkbox change it to the appropriate weeks for Track
Two.

Add Checkboxes. Change it to Track Two Individuals Dates. Add
the appropriate days for Track Two and change it to days using the first three
letters of the month.

Add Dropdown. Change it to If you selected by day, how
many days in the track did you select?. In chocies label the fields
in the following , 1 to 4 days, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15,
16, 17, 18, 19, 20.

Add Multiple Choice. Change it to Payment Options. Change the
multiple choices to Online Payment and Pay at SportHQ.

Add a Page Break.
33.
Repeat Steps 27-32 for Track One, Track Four, Three, and Traditional
(Winter Break).
