I am issuing this letter of warning because of your excessive absenteeism and your
failure to follow our companys attendance standards. On ___________(Date), you did
not report to work. General attendance rules require employees not to exceed 1.5
absences in a month.
You and I have previously discussed attendance. When you joined our company, I went
over my expectations on attendance with you. We talked about this issue again
on_____________, when I expressed concern over your frequent absences. On the

The company expects you to improve your attendance to an acceptable level on an
