As outlined in the syllabus, each class member is to provide a confidential evaluation of the members of their learning team. Your team assignment will
not be considered complete until all team members submit an evaluation. Please make every effort to evaluate each team member as accurately as
possible. This evaluation must be completed individually. These evaluations will remain completely confidential. Your Phase submission will not be
considered complete until I receive a completed evaluation form from each team member on your team.
