accordance with law, depose and say:

That I am the illegimate child of Evelyn M. Caete (mother), who is

currently working abroad;

2.

That I was born on October 08, 1995 at Dr. Jose Fabella Memorial.

Hospital, Lope Vega, Sta. Cruz, Manila;

3.

That on the date of my birth, we are living in Bagong Lipunan St., Phase

3, Las Pias, Metro Manila; and

4.

That my record of birth is registered in the local registry of the above-

mentioned place; and

5.

That I cannot personally file the supplemental report of my certificate of

live birth in the said local civil registry because I am currently living in Lapu-lapu City.
