I, HEDITA DE MANUEL TEMPORADA ANDRADE, Filipino, of
legal age, and a resident of 745 N. Roldan St., Kalibo, Aklan,
after having been duly sworn to in accordance with law, do
hereby depose and state that:
1. That I am the biological sister of SOLITA DE MANUEL
TEMPORADA, who was born on February 25, 1948 at
Navitas, Numancia, Aklan and died at Bugasongan, Lezo,
Aklan on February 24, 2021;
2. That my sister, SOLITA DE MANUEL TEMPORADA have
no legal spouse and children;
3. That I am the only nearest surviving kin of SOLITA DE
MANUEL TEMPORADA in Kalibo, Aklan to process and
receive her Death Certificate from the Philippine
Statistics Authority (PSA);
4. That I am executing this affidavit to attest to the
truthfulness of the foregoing statements and for any legal
purpose it may serve.
