AFFIDAVIT OF WAIVER OF RIGHTS

I, ARNALDO LEGASPI of legal age, Filipino Citizen, and a resident of Kalibo,
Aklan, after being duly sworn in accordance with the law, do hereby depose
and say:
That I am the father of KAITLYN T. LEGASPI a Grade Four student of
Kalibo Pilot Elementary School;
That my daughter is a member of the Women’s Artistic Gymnastics
Elementary in the coming 2023 Western Visayas Regional Athletic Association
(WVRAA) Meet;
That the players are required to be trained and housed in and be under
the care of the Department of Education Division of Aklan from from April 9 to
30, 2023;
That my daughter will not avail of the accommodation prepared for the
WVRAA housing during thee nigh time from April 9-30, 2023 but will be
spending her sleep time in our residence at
That my son will be trained on the training venues mentioned in the
training matrix submitted: (Tangalan Elementary School);
That we will abide by the schedules and activities in the training matrix
prepared by the WVRAA Committee;
That to the extent allowed by the laws and rules, I hereby agree to waive,
release, and discharged from any and all claims, causes of actions, damages,
and rights against the Department of Education Division of Aklan, the school
and its personnel relative to the conduct of this activity;
That I am executing this affidavit to attest to the truth of the foregoing,
willingly and voluntarily.
