AFFIDAVIT OF CONSENT

I, JEFFREY B. BATAN, of legal age, Filipino citizen and a resident of

Dalaoig, Alcala, Cagayan, do hereby solemnly affirm and declare as under:

1. That I am the cousin of ELMER BARTOLOME JR., who is applying for a

Non-Professional Driver’s License at the Land Transportation Office
(LTO);

2. That on behalf of his parents, I am giving my full consent to the said

child in his application for a Student Non-Professional Driver’s License;

3. That there is no criminal case pending in court against him nor has he been
charged of subversion, rebellion or insurrection in any court of the
Philippines;

4. That I am executing this affidavit to declare the truth of the foregoing facts as to my
undertaking to give consent to the child's application for Non-Professional
Driver’s License;
