AFFIDAVIT OF CORRECTION

I, PRINCESS XYRILL S. ANDENG, of legal age, married, Filipino

Citizen, and with residence at Poblacion, Baggao, Cagayan, after being duly
sworn to in accordance with law, hereby deposes and states:

1. That I am the biological mother of MAXRYLEV YROZAKI ANDENG

DUQUE who was born on November 6, 2021 Saint Philomene Birthing
Center, Pallua Branch, Tuguegarao City;

2. That in his Certificate of Live Birth issued by the Office of the Civil
Registry of Tuguegarao City, his name was erroneously recorded as
“MAX KALEB YROZAKI A. DUQUE”;

3. That his true and correct name is MAXRYLEV YROZAKI A. DUQUE;

4. That the names MAXRYLEV YROZAKI A. DUQUE and MAX KALEB

YROZAKI A. DUQUE, refer to one and the same person;

5. That I am executing this Affidavit to attest to the truth of the foregoing

facts and to support my request for the correction of his first name.
