AFFIDAVIT OF HEIRSHIP

I, JAYNON PUCOT TAGALOG, of legal age, Filipino, married and a resident of Purok Tugas 1,
Cabadiangan, Liloan, Cebu, after having been sworn to in accordance with law hereby depose and states
that:

1. I am one of the surviving heirs of my father, Barsabas Tagalog who was born on December 11, 1957 at
Cabadiangan, Liloan, Cebu.
2. Barsabas Tagalog and Wendel Tagalog are one of the children of the late Anastacio Tagalog and
Macaria Tagalog who died on January 5, 1992 and May 12, 1998 in Cabadiangan, Liloan, Cebu.
3. That I am executing the affidavit in order to attest the fact and truthfulness of the foregoing statements
and in order to support my claim before the office of Visayan Electric Company (VECO) and for
whatever legal purpose this may serve.
