AFFIDAVIT

I, ANGELA V. DELA CRUZ, of legal age, single, Filipino citizen,

and a resident of Centro, Amulung, Cagayan, after being duly sworn to
in accordance with law, hereby depose and state:

1. That I was born on November 13, 1999 at Concepcion, Iloilo to

spouses ALLAN DELA CRUZ and EDNA VELOSO;

2. That when I was still 5 months old, my parents abandoned me

after they have decided to live separately;

3. That they left my custody from my grandmother, LENALINE

MUYCO;

4. That since the abandonment up to the present, I had no

communication with them and haven’t receive any financial
support;

5. I am executing this affidavit to attest to the truth of the

foregoing statements and for whatever legal purpose it may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 15th day
