AFFIDAVIT OF DISCREPANCY

I, ESTELA C. SANTOS vda de CRUZ, of legal age, Filipino,

widow, and a resident of San Juan. Hagonoy, Bulacan after being
duly sworn to, hereby depose and state that:

1. I am the wife of Maximino C. Cruz and we got married on

August 22, 1954;

2. After verification of my Marriage Contract and my Certificate

of Live Birth, I discovered that my first name was misspelled as
ESTELLA instead of ESTELLA in my said marriage contract;

3. My true and correct first name is ESTELA as it same name

that I have been using in all my dealings, personal or official.
Likewise, it is the same name that I have been known in the
community;

4. ESTELLA appearing in my Marriage Contract and ESTELA

are one and the same person;

5. I am executing this to attest to the truth of the foregoing and

for whatever purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature
