AFFIDAVIT OF DISCREPANCY

I, JULIA S. LAURENTE of legal age, Filipino, married and a resident of Brgy. San
Antonio, Municipality of Tukuran, Zamboanga Del Sur, after having been sworn to in
accordance with law hereby depose and state that;

1. My name, as indicated in my Marriage Contract issued by the Local Civil Registrar, is

JULIA S. LAURENTE;

2. The last name indicated in the Philippine Identification Card is LAURINTE which
was inadvertently processed, and was a mere typographical error;

3. I am executing this affidavit to attest to the truthfulness of the foregoing and clear
the discrepancy that may arise from the document bearing the last name
LAURINTE to “LAURENTE” for whatever legal intents and purposes it may
serve.

IN WITNESS WHEREOF, I have hereunto set my hands this 5th day of September 2023 at
