SUPPORTING AFFIDAVIT

I, FLORIDEL S. LAURIO, Filipino, of legal age, single, and a

resident of Labangal, General Santos City, after having been sworn in
accordance with law, depose and say that:

1. I am a legal secretary of Atty. Joel Jonn U. Jumalon;

2. On April 13, 2023, I personally served a copy of the notice of

dishonor/demand for payment at the office of Joel Anasco Melendres No.
85 2/F J&G Propiedad, Nuñez St., Purok Malakas, San Isidro, General
Santos City, on April 13, 2023;

3. I also mailed another copy through LBC private courier on the same date
to his address at Block 10 Lot 13 Sunrise Village, Pagadian City, covered
by LBC Official Receipt No. DAP0181312, dated April 13, 2023;

4. I am executing this Supporting Affidavit to attest to the truthfulness of

the foregoing statements and for whatever legal purpose this may serve.
