AFFIDAVIT OF ACKNOWLEDGMENT/ADMISSION OF PATERNITY

I, CHERIN A. CARAG, of legal age, Filipino, single and presently residing at Barangay
Balug, Tumauini, Isabela, and after being duly sworn to in accordance with law, depose and
state:

1. That I am the biological father of BRIDNIE ALCANTARA who was born on April 22,
2001, in Barangay Balug, Tumauini, Isabela;
2. That at the time of her birth, I was not legally married to her biological mother Juvelyn A.
Carag;
3. That I hereby acknowledge my paternity of Bridnie Alcantara;
4. That I am giving my consent for her to use my surname CARAG and the same be
reflected in her birth certificate.

That I am executing this Affidavit to attest to the truth of foregoing facts and for whatever
legal purpose it may serve.
