state 2 id markers-name and DOB-each and every time

-I will be able to give medication

to two patients in one shift

-Double check medicine date and time.

Scheduling my clinical day

To have all my vitals, medications and clients ADL done before 9am
so as to take my breaks and have all my NC done before the end of
shift conference.

- take time to make a daily plan

-I will take my breaks on time

-review plan with my peers

-I will be ready for conference

Before the end of 14th week of

-start charting with completion of each client care

-review my plan at the end of the day so as to see where I
can improve and make necessary change
