Learning goal 1

Describe an area in your practice you want to further develop.

Since I have limited clinical experience at the hospital, I feel that I am not confident and experienced in
giving and receiving shift handoff reports which is something that I would like to develop.

Define your learning goal. What is it that you want/need to learn? Include a timeline for when you want to achieve this goal.

I will demonstrate improved confidence and competence in giving and receiving shift handoff reports.
This will be validated by my preceptor and assigned nurse during their evaluation of my clinical skills by
the end of this clinical period in April.

Describe the learning activities you plan to complete to achieve your goal. Include a timeline to stay on track.

1. Review the necessary components of SBAR to get familiar with what information I should be
prepared to know when giving shift handoff reports by February 3rd
2. Practice with my peers and colleagues by doing mock handoffs to get familiar with terminology and
style during and/or outside of clinical time by February 17 th
3. Observe RNs when the receive and give shift handoff reports at the beginning and end of shifts until
April 6th

Learning goal 2

Describe an area in your practice you want to further develop.

Another area that I would like to further develop in my practice is medicine administration. I have never
administered oral medications or injections to a real patient as I have only practiced on mannequins
during labs, so this is something that I will need further practice and experience.

Define your learning goal. What is it that you want/need to learn? Include a timeline for when you want to achieve this goal.

I will gain experience and skill on giving oral medications and injection medications to patients in a
proper and safe manner. I will observe RNs on the unit administering medications and ask appropriate
questions and tips they have from their experience. I hope to achieve this goal by the end of the
clinical semester so that I am able to administer medications safely and confidently with little to no
assistance.

Describe the learning activities you plan to complete to achieve your goal. Include a timeline to stay on track.

1. Practice different types of injections in the Trent Hub to gain practice of administering by March 3.
2. Review the commonly given medications on my unit and create appropriate drug cards so I gain
knowledge of the therapeutic use, side effects, etc. of each drug by March 10.
3. Know and practice your landmarks for IV vs PO vs IM vs SQ and ensure my understanding when one
is more appropriate than the others by March 10.

Learning goal 3

Describe an area in your practice you want to further develop.

Another area in my practice that I would like to further develop is completing comprehensive patient
assessments so that I can create an appropriate nursing care plan to implement during my care of that
patient.

I will be able to provide nursing care that includes comprehensive, collaborative assessment, analysis,
planning, evidence- informed interventions and outcome measures. By doing so, I will be able to
provide the best care possible to my patient so they can have good health outcomes. I hope to achieve
this goal by the end of the clinical period in April.

1.Review patient reports on EPIC before so that I have an accurate picture of their health (medical
history, allergies, family history, etc.) – I will do this at the beginning of each clinical shift
2.Create a nursing care plan for my patient(s) to gain practice during my clinical which will have a
detailed report on the patient – I will do every clinical week for each patient that I will be caring for
throughout the semester
3.I will engage with patients and their families to understand and identify their health/personal goals are
related to their illness or reason for being admitted to the hospital so that we can work towards that
goal – I will do this every clinical I attend throughout the semester

Learning goal 4

Describe an area in your practice you want to further develop.

An area in my practice that I would like to further develop is creating a therapeutic relationship with my
patients and their families to provide the best care possible.

Define your learning goal. What is it that you want/need to learn? Include a timeline for when you want to achieve this goal.

I will demonstrate that I have the ability to establish and maintains therapeutic, caring and culturally safe
relationships through effective communication. Through this effective communication, I would like to
understand the patient’s experience of acute illness and their bio-psycho-social needs so that I can
apply it my nursing plan of care and advocate for the accordingly. I hope to achieve this goal by the end
of the clinical period this semester (April).

1. Review RNAO’s best practice guidelines for establishing therapeutic relationships by Feb 3 rd
2. Introduce myself to my patients at the beginning of each shift and try to spend some quality time
getting to know them so I can understand them – I will do this each clinical shift
3. Read 2 scholarly articles about developing effective and caring nurse-patient relationships by Feb
10th
