Goals & Objectives

Goals
Form a community based program to provide comprehensive education encompassing end of life
matters that is centrally located and easily accessible for community members.
Give patients and families the ability to make an educated decision on end of life care.
Increase the awareness for the need of end of life planning and demonstrate the benefits of
having a plan in place before a person is unable to make these decisions for themselves.
Allow access to end of life planning materials to people of all ages in the general public setting.
Provide a non-threatening environment for families and patients to discuss feelings surrounding
end of life matters outside of the acute care setting.

Objectives
To enhance community

Activities

avenues of advertising that
will reach the population
we are attempting to target.

advertising we would like
to pursue (i.e. newspaper

brochures, bulletin boards,
etc.).

impactful and inspire the
general public in advance
care planning.

Excellent! I really like the table format. If it is permissable, I would like to


keep a copy of this and use as an example for future cohorts. This is the
best 1st submission I have had in my 4 years of teaching this course!
