Applying to Emagia Corporation for Trade Credit

In order for Emagia Corporation to extend credit to your business for the purchase of our goods and services,
you must submit a credit application through this website. As you complete the application, you will be asked
to provide certain information about your business. Your application cannot be processed unless all of the
requested information has been entered. Because your web browser will "time-out" if no information
has been entered within a five minute period, you should gather all of the necessary information
before beginning the application process. That information will include:
1) your name and title, the legal name of your business and any other names your business uses, the date
your business was established, the address, telephone number, fax number, email address, and federal tax
identification number of your business;
2) the names, addresses and social security numbers of the principal owners or officers of your business;
3) the name of your bank, the name of your contact at that bank, the telephone number for that contact, and
your account number at that bank;
4) the names, addresses, telephone numbers and fax numbers of four trade references (you should notify
those references that Emagia Corporation might be contacting them); and
5) your financial statement (only required where the credit requested would exceed $25,000) and a sales tax
exemption certificate (if applicable).

Your submission of a credit application through this website shall be deemed evidence as to your agreement,
on behalf of your business, to conduct transactions with Emagia Corporation by electronic means, as that
term is used in the Uniform Electronic Transaction Act.
Your submission of a credit application through this website shall be considered your agreement, on behalf of
your business, that Emagia Corporation's Standard Terms and Conditions, which are set forth below, shall
apply to the sale of any goods and/or services by Emagia Corporation to your business. You will be asked to
specifically acknowledge that agreement when you submit your application.
