Two forms of workplace monitoring are:
- Active systems (Proactive) which monitor the design, development, installation
and operation of management arrangements, RCSs and workplace precautions;
- reactive systems which monitor accidents, ill health, incidents and other
evidence of deficient health and safety performance.

2. Give five types of proactive monitoring in the workplace.

3. Outline the objectives of reactive monitoring.
Measure negative outcomes from the organisation and provide opportunities for
improvement

5. Define the term `health and safety audit.
An audit is a systematic, critical examination of an organisations systems to
determine the extent of compliance with a set of agreed standards.
6. Outline the reasons for investigating accidents.
Legal reasons to investigate accidents to prevent reoccurrence, there are specific
legal requirements for reporting of accidents, ill-health, dangerous occurrences
to competent authorities.
7. Provide a definition of the term `Dangerous occurrence.

Dangerous occurrence: Readily identifiable event as defined under national laws


and regulations, with potential to cause an injury or disease to persons at work
or the public.

8. Wirth reference to identifying immediate causes of an accident outline unsafe


acts and unsafe conditions.
Unsafe Acts
Unsafe acts are made by people; they are not made by systems or by faulty
equipment. People make unsafe acts.
Unsafe conditions
Unsafe conditions in the workplace can be mechanical or physical e.g. an
electrical cable across a walkway presenting a trip hazard.
9. Identify the types of report forms that could be utilised following an accident.

10. Outline the purpose of reviewing an organisations health and safety


performance.
According to the HSE the purpose is that "the organisation learns from all
relevant experience and applies the lessons". So basically the organisation is
reviewing how effective their system is and how it can be improved, should it
need to be.
