b) What is the goal of treatment and what type of therapy is indicated for

AB at this time? Explain it.


The goal of therapy is to achieve complete clinical and hematologic remission which
means complete disappearance of all physical and bone marrow evidence. Normal
hematopoiesis have to be maintained to prevent bleeding, infection and to make
sure patient is not in harmful condition.
Besides that, the goal is to decrease relapse rate in patient, prolong complete
remission and improve event free survival rate as well as the overall survival.
Type of therapy,
-

Myelosuppresive chemotherapy.
WBC level influences the induction therapy

Treatment induction ( low/ intermediate risk)


1) ATRA + Arsenic trioxide 0.15mg/kg/day
OR
2) ATRA + Daunorubicin 50mg/m2 4 days + Cytarabine 200mg/m2 7days
OR
3) ATRA + Daunorubicin 50mg/ m2 3days + Cytarabine 200mg/m2 7days
OR
4) ATRA + Idarubicin 12mg/m2 on day 2,4,6,8

High risk
1) ATRA + Daunorubicin 50mg/ m2 4 days + cytarabine 200mg/m2 7days
OR
2) ATRA + Daunorubicin 60mg /m2 3days + cytarabine 200mg/m2 7 days
OR
3) ATRA + Idarubicin 12mg/ m2 on day 2,4,6,8
OR
4) ATRA + age adjusted Idarubicin 6-12mg/m2 + Arsenic trioxide 0.15mg/kg/day
