Transition Planning Meeting Notes June 11, 2015

Purpose: Incoming students from MMS in grade 6 and 7. These students


have been reassigned to our middle school due to their school being closed
due to declining enrollment.
Participants: M. Cosman EST-Resource, M. Leach EST-Resource, P. Sleep
EST- Guidance
Students with Medical, Academic, Behavior needs currently in grade
7
Louis A. - academic and behavior needs, socially inappropriate, manipulates
others and does little class work and assignments
Cassie B. medical/emotional needs, high social anxiety to the point of
shaking scared, involved with guidance
Andre C. social/communication/behavioral needs, ASD diagnosis but very
high functioning, can become easily distracted (seating arrangement, head
phones for independent work) hard to get focus back, has a loud voice,
intelligent and very social
Chase E. emotional/academic, very needy, constantly in personal space,
socially awkward, trouble fitting in to a peer group, involved with guidance
Jon G. PLP/Accommodated but still struggling academically as well as
socially. Currently residing in a group home. He has no connections at school
and has violent interests. Keep close tabs.
Danica L. POLICY 704 latex allergy as well as cold weather
Students with Medical, Social/Emotional , Behavior, Academic needs
currently in grade 6
Ella G. emotional needs, has to complete work before moving on, socially
connected
Mike S. Academic needs, follows a PLP-modified for L.A. and Math, loves
playing sports
Troy W. Academic needs, follows a PLP-Modified for L.A. and Math, needs
classroom support
Aliyah P. medical needs, has a benign brain tumor but is unaware of
diagnosis and has frequent appointments. She loves sports and clubs
