Accident Communication: Accident While Cutting Down A Tree At The Football

Description:
4 contract workers were assigned to cut dow a tree in the football field. A work
permit was obtained and safety briefing was given to the 3 contract workers and
their contract supervisor by the safety officer. The workers began their work at
about 10.05 am.
At one point, one of the man holding to the rope left and only one men was
holding on the rope. The man on the tree sawing the branch. The branch fell and
was entangled to some branches below. The one man holding on the rope lost the
grip of the rope and the cut branch slipped horizontally toward the men on the
tree. The edge of the cut branch pinned the right food on the man who was
cutting the branch of the tree,
The other man on the tree assisited the injured to climb down the tree and the
injured was taken to the factory cline for first aid treatment. After that, the
injured was sent to Hospital Batu Pahat.

Finding/Issues:
1. Even though during safety briefing the contracts supervisor ensured that
his workers had experience in tree cutting, the men engaged in the tree
cutting were not skilful. This was visible when they performed the job.
2. The worker were not well equipped and no proper tools to perform the job.
They came without safety helmets, no ladders, 2 men with slippers, no
proper rope and the chain saw was too small and had a starting problem.
3. Selection of contractor and their workers was not done properly and it was
left to the contractor of how they do the job. No coordination and
supervision from the GA section.

Cause of Accident:
1. Failure to secure the cut branch falls down in the planned direction. Not
enough of men force and the rope were not long enough. The rope was
made of nylon and the men had no hand gloves.
